CYSHCN Presenation is available to the public community!

 

The Children & Youth with Special Health Care Needs Program at Henry County health Center is now offering presentation to the communities. Our program not only provides service in Henry County but serves Bates, Benton, Cedar, Hickory, Polk, St. Clair and Vernon Counties.

 

We welcome the public to request a presentation of our program. We would be happy to come to your organization to show our information.

The CYSHCN Program (Children and Youth with Special Healthcare Needs) provides assistance statewide for children birth to age twenty-one who meet medical and/or financial eligibility guidelines.  The Program focuses on early identification and service coordination of children with special health care needs and their family.  For the purposes of this Program, children with special health care needs are those children who have, or are at increased risk for, a disease, defect, or medical condition that may hinder the achievement of normal physical growth and development.
 
Service coordination facilitates, implements, coordinates, monitors and evaluates services and outcomes, and encourage participants/families to develop skills needed to function at their maximum level of independence.
 
The Program provides two primary services:
 
1.  Program service coordination, which includes:
  • Outreach activities and early identification of participants,
  • Eligibility determination,
  • Assessment of needs,
  • Coordination of services through resource identification and referral,
  • Family support,
  • Service plan development/implementation,
  • Monitoring and evaluation
  • Transition to community-based services

2. Supplemental funding for services which may include:

  • Doctor and specialist visits
  • Hospitalization
  • Therapies
  • Prescription Drugs
  • Durable Medical Equipment
 
Service Coordinator covers the following counties: Bates, Benton, Cedar, Henry, Hickory, Polk, St. Clair and Vernon Counties
 
Eligibility Requirements:
Be a resident of the State of Missouri
Be under the age of 21 (twenty one)
Have an eligible special health care need, and
Meet financial guidelines for supplemental funding
